ABSTRACT
Introduction: Perianal fistula is a rare disease and can recur even after surgery. There are various
surgical techniques to treat perianal fistulas, but they have a high risk of causing complications of
fecal incontinence and worsening the sufferer's quality of life.
Case report: This report describes the case of a 55 year old male patient with a lump in the area
around the left anus that had been present for 2 weeks. The lump is felt to burst and release pus
every time the patient finishes defecating. The patient has experienced similar complaints
repeatedly since 2004 but has always left it until the lump bursts by itself. The lump always appears
in the same area.
Conclusion: Surgical treatment using the fistulectomy technique has a good prognosis even though
in some cases it is considered to have a high risk of fecal incontinence, making it a challenge for
surgeons to choose the right management.

ABSTRAK
Pendahuluan : Fistula perianal merupakan penyakit yang jarang terjadi dan dapat timbul berulang
meski telah dioperasi. Terdapat berbagai teknik pembedahan untuk menangani fistula perianal,
namun berisiko tinggi menyebabkan komplikasi inkontinensia fekal dan memperburuk kualitas
hidup penderita.
Laporan kasus : Laporan ini memaparkan kasus pasien laki-laki usia 55 tahun dengan benjolan
pada area sekitar dubur sebelah kiri yang dialami sejak 2 minggu. Benjolan dirasakan pecah dan
mengeluarkan nanah setiap pasien selesai buang air besar. Pasien mengalami keluhan serupa sudah
berulang kali sejak tahun 2004 namun selalu dibiarkan hingga benjolan pecah dengan sendirinya.
Benjolan selalu muncul di area yang sama.
Kesimpulan : Tatalaksana pembedahan dengan teknik fistulektomi memiliki prognosis yang baik
meski pada beberapa kasus dianggap memiliki risiko tinggi inkontinensia fekal, sehingga menjadi
tantangan bagi ahli bedah untuk memilih manajemen yang tepat.
