STATEMENT OF UNDERSTANDING

By signing below, I acknowledge and declare that I have understood all materials provided to me in
conjunction with my application to Tulane Graduate Direct, including any agreements, statements, or
other documents to which I have affixed my signature. To the extent that I have authorized a third
party, including my agent, to sign any part of my application to Tulane Graduate Direct, on my behalf, I
acknowledge that I have read and understood all such documents submitted. I am aware of the
binding nature of any agreements, including my program’s terms and conditions and the refund
provisions contained therein, signed by me or by my authorized representative. I have disclosed all
post-secondary credits earned by me to date.

I agree that I am responsible for all information and representations that appear on my application, and
certify that the information I have supplied is complete and truthful.
