PARENTAL CONSENT
Every volunteer under the age of 18 and above must ask their parents or
guardian to complete and sign this form and please return the completed
parental consent form to The Runaway Volunteer Admin. Please do not start
volunteering until you have returned this form.

I/We hereby willingly and voluntarily give consent the participation of my/our
son/daughter, ___________________________________, in the area or program
where she/he chooses to volunteer in the Runaway Volunteers.

I have considered the benefits that my son or daughter will derive from his/her
participation in your activity provided that due care and precaution will be
observed to ensure the comfort and safety of my son/ daughter and the
Runaways staff and personnel may not be held responsible for any untoward
incident that may happen beyond their control.
