We are 3rd Year BS International Hospitality Management with Specialization in Cruise Line
Operations in Culinary Arts from the Lyceum of the Philippines University Manila. We are
currently conducting a research entitled "PLACE TITLE HERE." This study seeks to PLACE
OBJECTIVES OF THE STUDY HERE.

In line with this, may we request you to become one of our jurors to provide quantitative and
qualitative review of the research instrument. This is the first phase of our research questionnaire
validation. For this step of the validation, the intention is to check each item in terms of
relevancy to the construct underlying study as per the theoretical definitions of the construct
