Michael states that on Monday the 10th of September 2018 Client Yazmin Brodie, Support Worker
Jade and Team Leader Janet were in the residential kitchen area, further; it was stated that Support
Worker Michael was removing himself from the kitchen as client Yazmin had stated “I fucking hate
you cunt” “ I am going to make sure that you get fired” Support Worker Michael states that Client
Yazmin then has screamed at him “I am going tell child safety that you touched me down there”.
Support Worker Michael has then turned to support worker Jade and Team Leader Janet and asked
“Did you hear that” Support Worker Michael then states he did not get a response as he went
straight into the office to contact Service Manager Deb, however; on the way to the office Client
Yazmin threw a cup at support worker Michael hitting him in the back of the neck, it was stated that
the contents of the cup went all over Michael, the walls, desk and Laptop located inside of the
residential, further; Support Worker Michael states that Service Manager Debra agreed with him
leaving the residential, Support worker Michael states that he left the premise. (Support Worker
Michael stated to his colleagues that he is not putting up with this fucking shit)

Michael states that on Tuesday 11th of September 2018 he was contacted from a Southern Cross
Support Services reprehensive, however; unable to remember who it exactly it was advising that he
was not required to attend his shift. Michael states that on Tuesday the 11th of September 2018 he
received a call from Service Manager Debra which was checking on him / offering support, however;
Michael states that he was fine and did not require support.

Michael states on Wednesday 12th of September 2018 that he arrived at 57 Koowin Drive, Kirkwood,
Support Worker Michael states that Client Yazmin approached him at the front and stated, “What
the fuck are you doing here” “Fuck off I don’t want you here go home”. Support Worker Michael
then commented on how lovely Client Yazmin’s hair looked as he noticed that she had it cut,
however; Client Yazmin replied and stated “Get Fucked” Support Worker Michael states that he was
having a smoke at the bottom of the driveway before commencement of shift with Support Jeanine,
Team Leader Janet had left the residential to go and collect Client Isaac so Support Worker Michael
could take him to the skate park for the afternoon and sperate client Yazmin and Client Isaac.

Michael states that when Client Isaac arrived at the residential he requested assistance from Support
Worker Michael to help fix his scooter, further; Support Worker Michael states that he was squatting
down whilst Client Isaac was holding the handle bars of the scooter, Michael states that the garage
door entrance was at the back of him. Support Worker Michael states that Client Yazmin came into
the garage area and then poured milk all over Support Worker Michael, further; Support Worker
Michael then states that he stood up from squatting position and placed his hands around the milk
carton and squeezed the remaining contents toward Client Yazmin, Support Worker Michael states
that he kept the milk carton in his right and had client Yazmin blocked with his left open hand on her
chest plate, further; Support Worker Michael states that he threw the carton on the ground as Client
Yazmin continued to swing with her arms and kicking at Support Worker Michael. Team Leader
Janet came running up the drive way yelling out “No No Michael Stop” further; Support Worker
Michael states that he had stopped when Team Leader Janet instructed him to do so.

Support Worker Michael states that he was not intending to cause any harm to Client Yazmin
throughout this situation.

Support Worker Michael states that Client Yazmin took off into the kitchen area of the residential,
further; Support Worker Michael heard stuff smashing off a bench, Support Worker Michael states
that Team Leader Janet and Support Worker Jeanie all were looking at each other in shock as they
had all heard a horrendous smash from inside of the house.

Support Worker Michael states that Client Yazmin then came charging out at Support Worker
Michael with a dinner room chair, Team leader Janet attempted to intervene by taking the chair off
Client Yazmin which was intermediately successful, however; Client Yazmin has then used her body
to charge at Support Worker Michael bowling him over whilst she had hold of the top of his shirt
pulling him to the ground which resulted in extreme pain, Support Worker Michael has landed
extremely hard to his left hand side of his body. Client Yazmin then continued to kick and hit Michael
with all her arms and legs, further; Support Worker Michael then states he had been kicked multiple
times to the chest area. Team Leader Janet and Support Worker Jeanine were screaming “Get Away
Michael” “Try and get away”, further; Support Worker Michael states at this point he was
attempting to get away which has resulted in him slipping and sliding over the concrete floor that
had liquid all over it from the milk spilt previously, further; Support Worker Michael states that Team
Leader Janet was able to hug Client Yazmin from behind to usher her away from Support Worker
Michael, Michael states as he was getting up he did make contact with Client Yazmin due to her
repeatedly trying to assault him and Michael not being able to support himself safely to get away.

Support Worker Jeanine advised Michael that he was to get his belongings and leave the residential
straight away, further; Support Worker Jeanine then advised Support Worker Michael that he was
not able to leave as he needed to wait for Emergency Services to arrive. Support Worker Michael
states that Support Worker Jeanine had then locked him in the office to keep him safe, Michael
states that Team Leader Janet and Support Worker Jeanine provided immediate response to ensure
that Client Yazmin was okay. Support Worker Michael has stated that he overheard conversations
with Yazmin speaking with her father, he states that when the call had ended Client Yazmin
screamed out to Support Worker Michael that her father was coming around to slit his throat.
Support Worker Michael states that Client Yazmin’s father and his girlfriend attended the Koowin
Residential.

Support Worker Michael states that Queensland Police & Ambulance Services arrived at 57 Koowin
Drive, Kirkwood. Michael states that Queensland Police had asked him if he would like to press
charges and at this time Michael had agreed to go ahead with this, further; Michael states that
Queensland Ambulance Service came in to the office and provided medical attention, Michael states
that he was then transferred to Gladstone Public Hospital.
