NHPPD( standard nursing time)

• The number of nursing care hours relative to the


patient workload per patient per day

• Ranges from 3-7.5 hours depending on unit type


(general ward,…….. on day surgery , HDU, ICU )

• The average amount of time used by nurses to


provide care for a pt in a ward .

Nursing care hour
• The number of productive hours worked by nursing
staff assigned to the unit who have direct patient
care responsibilities for greater than 50% of their shift.

Total nursing hours

• The total amount of time used by nurses to provide

care for pts per day in a ward


o TNH =Number of Beds x Occupancy Rate x Standard Nursing Time
OR
o TNH= ADC*NHPPD

Productive Hours:

• Actual direct hours worked, not budgeted or

scheduled hours

• Excludes vacation, sick time, orientation, education

leave, or committee time.

Patient Days

• Number of patients who were occupying a bed as


of midnight.

• The day the patient is admitted is a patient day

• A ‘patient day equivalent’ is equal to ONE inpatient


bed day
• One patient occupying one bed equals one day

• ==one overnight stay by one patient) or three outpatient visits


or three emergency visits or three emergency maternity
attendances or two deliveries==

Average Daily Census (ADC)
The census report describes on a daily basis the number of
patients who were occupying a bed as of midnight. One
patient occupying one bed equals one day

The ADC reflects the average number of patients per day.

ADC = # of Pt. Days in a given time period


# of days in a given time period

EXAMPLE: if a census report for a 4 week (30 days) time period


shows the number of patient days is 450, then the ADC would
equal 15 .

Total patient days of care
• Sum of each daily inpatient census for the time
period examined.
• For instance, if the time period examined was a
week, and the daily inpatient census was as follows:
Day 1=30, Day 2=28, Day 3=26, Day 4=35, Day 5=35, Day 6=25,
Day 7=25, then the total inpatient days of care for
that week would be 30 + 28 + 26 + 35 + 35 + 25 + 25
or 204 total inpatient days.

Occupancy
• The ratio of the ADC compared to the number
of beds available:
Occupancy = ADC in time period
# of available beds in time period

EXAMPLE: If you have a 20 bed unit and your


ADC is 15, your occupancy rate is 75%

Average Length of Stay (ALOS)
• Length of stay equals the number of days a
patient is hospitalized from admission through
discharge.

ALOS = # of pt. days in a given time period

Considerations
Every effort should be made to strive for the
lowest length of stay possible.

25 bed unit with 662 patient days and 92 discharges in


a month

18 bed unit with 1285 patient days and 102 discharges


in a quarter

21 bed unit with 6900 patient days and 1408 discharges

• 25 bed unit with 662 patient days and 92


discharges in a month
o 662 days/92 discharges = 7.2

• 18 bed unit with 1285 patient days and 102


discharges in a quarter
o 1285 days/102 discharges = 12.6

• 21 bed unit with 6900 patient days and 1408


discharges in a quarter
o 6900 days/1408 discharges = 4.9

Nursing Hours Needed/ Day = 72 hours

Case 1

Unit: Internal Medicine

• Distribution of Nursing time : 70% Nurses/midwifes and 30%

Nursing/midwifes Assistants

• Nurses/midwifes Hours Needed= 0.70 x 72hours = 50.4 hours/day

• Nursing/midwifes Assistants Hours Needed = 0.30 x 72 = 21,6 hours/day
