I, Jocelyn Tanguan, hereby provide my full consent for my child, Jerry Tanguan, to
participate in the upcoming school activity scheduled to take place on January 29, 2024, at
Fort Bonifacio.

Having been informed of the nature and details of the activity, I willingly grant permission
for my child to partake in this event. I trust that the school and its dedicated staff will take
all necessary and reasonable precautions to ensure the safety, well-being, and supervision
of the participating student throughout out the duration of the activity.

In the event of any unforeseen circumstance or emergency that may arise during the
activity, I authorize the school to take any necessary measures to ensure the health and
safety of my child. I understand that the school will make every effort to contact me or the
emergency contact provided before seeking any required medical treatment.
