consent for my child to participate in the Industry Tour organized by BSA/BSBA
Faculty for the subject Operations Management and TQM. The details of the tour are
as follows:

● Date of the Tour: March 26, 2024 – Tuesday

● Itinerary:
11:00 AM - 12:00 PM Gathering at the campus
1:00 PM - 2:00 PM PROFOODS PLANT AND SHOWROOM VISIT
3:00 PM - 4:00 PM GARDENIA PLANT TOUR
3:30 PM- 5:00 PM Back to campus/End of Visit
● Purpose of the Tour: This tour will provide them with invaluable exposure to
real-world operational environments and students would gain practical insights
into various facets of business management, including production processes,
quality control measures and workforce dynamics.

I understand that my child will be under the supervision of school staff throughout the
duration of the tour. I authorize the school to seek medical treatment for my child in case of
emergency and I agree to be responsible for any associated medical expenses.

I acknowledge that my child must adhere to the school's code of conduct and behavioral
expectations during the tour. I will ensure that my child is adequately prepared for the trip,
including any necessary attire, supplies, and medications.

I have read and understood the itinerary, safety precautions, and any additional information
provided by the school regarding the tour. I consent to my child's participation and agree to
release the school and its staff from any liability arising from the educational tour.
