SUBJECT: INSPECTION REPORT ON NOTED DEFECTS OF NEWLY COMPLETED

PROJECT WITH CONTRACT ID NO. 23HG0036 – CONCRETING
ALONG NBA, BALISONG ARGAO, CEBU

As per site inspection conducted last January 9, 2024. Hereunder are the list of noted
defects/deficiencies on the above-mentioned project.

31. Large potholes have been developed between the gaps of existing old pavement
and the newly constructed concrete pavement.

Recommendation/s:

1. The Construction Section and the Project Engineer of this project should address
the said defects to the contractor for further repair/reblocking on various noted
defects and deficiencies.

I am hoping that the above-noted defects serve as enough evidence that the enumerated
sections are in need of immediate action. Failure to rectify the noted defects/deficiencies is a
valid reason for non-signing of the Certificate of Acceptance.
