On the day of your appointment, please come with:

> Your passport or any other travel document (containing at least two consecutive
blank pages) + photocopy of the identity page of this document and the last visas
obtained

> Originals + photocopy of all the documents listed below, translated into French
or English.

Attention: the absence of one or more of these documents may result in the
refusal of the requested visa. The competent visa department reserves the right to
request additional documents and information from you.

Prerequisites
Non-Filipino applicants: copy of the Alien Certificate Registration (ACR)
certificate valid at least three months after the date of departure from the
territory of the Member States.
Identity photography.
If you hold an official travel document, mandatory presentation of a note
verbale.
A travel document, issued less than 10 years ago, containing at least two
blank pages, with a validity period of at least 3 months greater than the date
on which you plan to leave the country; Schengen Area or, in the case of a
 long stay, with a validity period of at least three months greater than the 
expiry date of the requested visa. Be sure to transmit (scan) ALL PAGES of
your travel document containing visas, entry and exit stamps or any other
inscriptions.
Purpose of the trip/stay
Copy of the trip itinerary.
Detailed daily itinerary of the planned trip.
Proof of installation in country of residence
Proof of activity (certificate of employment, certificate of leave, latest
income tax return, proof of company registration issued by the DTI or
registration with the SEC, education certificate. ..) and proof of ties to the
Philippines (proof of payment of social security contributions, copy of title
deeds to real estate, proof of family ties to the Philippines: marriage or birth
certificates of children issued by the PSA).
Resources
Bank certificate, bank books, personal bank statements showing regular
income, credit card statements or balance for the last six months. For minors:
proof of financial resources from parents or legal guardian.
Accommodation
Confirmation of hotel reservation, hotel voucher or letter of promise of
accommodation by the host or certificate of welcome.
Travel medical insurance
Travel medical insurance certificate (see FAQ for details).
