Miss Latifah is a 19 years old teenage girl.

She was a SPM holder but having no experience of


working. She was diagnosed with Major Depressive Disorders (MDD). After she has MDD, she prefers
to stay at home, refuses to hang out with her friends and is not comfortable with activities outside
the home. Currently, Ms. Latifah lives at home with her parents because her sisters are studying at
university and her younger brother is studying at a boarding school. Miss Latifah’s mother stated
that she used to mingle around with her friends and once a bright student. Her date of onset started
years ago after she got her SPM results, she felt very sad with her grade because there were some
subjects for which she did not get excellent marks. Her parents were very disappointed because all
her siblings had gone on to study abroad but she could not because her grade did not pass the
requirements to go to a university abroad.

Since then, she has lost appetite, could not sleep at night, feeling worthless and has several suicidal
attempts. Miss Latifah had slightly scratched her wrist with a knife in the kitchen and was safely
stopped by her mother and rushed her to the near hospital because Miss Latifah was bleeding in her
wrist. After that, Miss Latifah was taken by her parents to get mental treatment in the hospital and
she has been diagnosed with Major Depressive Disorders (MDD) by the psychiatrist.Miss Latifah has
several problems in which she has poor insight, problems in social skills and communication, coping
skills and sometimes she has low self-esteem. For self-care activities and IADL, she can manage by
herself but needs assistance by her mother due to her lack of insight. At times, she just ignored her
personal hygiene and always locked herself in her room. Her parents were aware about her
condition but the other siblings considered her as a burden and embarrassing their whole family.
After Miss Latifah has finished her school years, her parents are now concerned about her ability to
survive outside and have a job to support herself. Her parents want Miss Latifah to be independent
in the future and not be completely dependent on them.

[More Information ]

She had history of admission into psychiatric ward for a week because of suicidal attempt. Her
mother claimed that her daughter always neglect her medication routine and only dependent on her
parents for medication routine. Her mother was very worried if her condition relapse. Currently, Ms
Latifah has been discharged from the psychiatry ward and being referred to Occupational Therapy
department at a hospital in Malacca as an outpatient. During the assessment process, she shared
her concern about her future self and wanted to have a continuous financial support and if possible
to avoid any relapse. She stated that she did not want to further study anymore. She wanted to be
productive and get a job. Due to her condition, she was afraid and worried if no employer wanted to
accept her. She also had no confidence to face other people because she thought that she is
worthless, not good enough and afraid of being judged by others. She also afraid to socialize with
others due to low self-esteem lack and did not know how to initiate a communication. She also had
no idea what to expect and job that she could do. She only had a hobby that she treasure which is
sewing. Before she was diagnosed, she loved sewing so much, but the hobby slowly diminished after
that. She wanted to explore new things that she could do. She also has problem in making decisions
and prioritize what she wanted and put too much thought on simple things. She could not sustain
attention in her tasks as she had many things running on her mind. These problems were affecting
her in daily lives activity. You are the occupational therapist that will facilitate Ms Latifah in
alleviating her symptoms and focusing on her work life
