FOR THE S.Y 2022-2023
Researchers: Rose Jane P. Jason, Shainly Mae A. Infanso, Jessie F. Orpio, Department of
Education, Hingatungan, Silago, Southern Leyte
The researchers would like to invite you to be a research participant in the research study
entitled: “Factor Analysis: Basis of Choosing Senior High School Strand among Grade 11
students of Hingatungan National High School for the S.Y 2022-2023.
You are chosen as one of the participants in this study because you fit the inclusion criteria
of the target participants of this study. Your participation will provide data for the researchers
to determine the different underlying factors which serve as the basis for choosing the senior
high school strand among Grade 11 students of Hingatungan National High School for the
SY 2022-2023. Your involvement in this study is voluntary. You are allowed to withdraw your
participation in this study whenever you wish to with no penalty or actions to be taken
against you. In the case of withdrawal of participation, the data you have contributed will be
withdrawn as well and will not be part of the data analysis of this study.
In assurance, there are no known risks concerning your participation. If you will approve my
request of having you as a participant in this study, we, the researchers, will be
administering a survey questionnaire for you to fill in. Your personal information will not be
mentioned in any documents of this research study. If you may have any questions with
regards to the research study, please feel free to contact us through the mobile number

information presented. You will be given a copy of this consent form to keep.
