We are pleased to announce that we will be having our Management outing on April 13, 2024 (Saturday) at Woodside Farm and
Water Park, Cabuyao, Laguna. Games and other activities will fill the day. For the confirmation of your attendance, please sign this
survey form.

As we need to finalize the contract with the resort, please make sure that your confirmation is FINAL. Confirmed employees who
decided to cancel his or her participation shall shoulder the cost related to this activity via salary deduction.

 Guidelines

1. Participants are expected to adhere to the standards of behavior outlined in the Code of Conduct
(COCD). Any acts that violate or constitute a breach of the policies specified in the COCD will be deemed a
violation and may result in disciplinary action. Such actions will be addressed in accordance with the
relevant provisions of the Green Cross COCD.

2. Respect and adhere to the activity rules and guidelines.

3. Act in courteous manner and treat other participants with respect. Appropriate language and behavior are
expected at all times.
4. Adherance to resort guidelines is a must

Behaviors not permitted:

1. Improper use of language
2. Destruction of property
3. Possession or use of harmful objects.
4. Possession of illegal drugs.
5. Stealing of other property
6. Bullying and being disrespectful
7. Aggressive physical behavior
8. Leaving the event without notifying or asking permission
9. Others

By acknowledging this form, I affirm my understanding that, through my signature, I willingly accept the
consequences for any actions taken in disregard of the Code of Conduct (COCD) and associated charges. I
am aware that failure to adhere to the COCD may result in repercussions for which I take full responsibility.

payroll account.

4 years old and above - 100%

Resort: Actual rates for immediate dependents will be emailed separately to employees once finalized.
