Patient profile

Mr. Ali Hassan, 45 years old male resident of Bahria town, Lahore / USA booked for FUT Scar Revision
and Nano-fat Stem Cells Transplant on 14 September, 2023 for Mini FUE session from head and beard
for FUT Scar Revision measuring 3cm on Right side from Previous Hair Transplant surgery, and closed in
double layered fashion with Vicryl (Dissolvable) sutures.

Previously, there were 2 sessions of Hair Transplant 2006 and 2022 respectively, first being uneventful
but in second session, there was delayed blood clotting.

Lab values
His Vit. B12 & Vit. D3 were low. His urine complete, coagulation profile, lipid profile, LFTs and
cholesterol level were normal. His platelet count was 120,000 (His Platelets count remains 90,000 to
120,000 range throughout his life.)

Procedure Day Assessment

In his Pre-op assessment his hair were fine having thin texture.

Surgery day; the patient was vitally stable. Inj. Ceftriaxone 500mg was given after test dose. Inj. B12 and
D3 was given.

Under aseptic measures FUE 749 hair follicle grafts placed in scar area extracted from beard and scalp.

Nano-fat Stem Cells Transplant: Under aseptic measures, approx. 800ml Tumescence anesthesia of
abdomen done through 1.5mm punch holes around umbilicus and waited for 1 hour. Then 40ml fat was
extracted and converted to Nano-fat using different filters and injected along PRP into the Scalp area.

Follow-up
Patient presented to us with bruises and superficial abrasions on abdomen extends from umbilicus
region to right and left flanks & also oozing of tumescence fluid from 1.5mm punch holes around
umbilicus.

On examination, abdomen was tender multiple clots were palpable. There was also superficial abrasions
of skin on abdomen due to pressure bandage. Bullous impetigo was also noted around superficial
abrasions. Otherwise, patient was vitally stable.

Following medications were started on daily basis:

 Inj. Ceftriaxone 1g BD
 Inj. Omeprazole 40mg OD.
 Polyfax Plus Ointment to be applied on abrasions TDS.
 Inj. Ketorolac SOS for Pain.

Following Blood tests ordered on daily basis:

CBC, Blood Cultures, LFT, RFT, Serum Electrolytes (All were unremarkable)
Conclusion:
Mr. Ali Hassan has recently undergone Scar Revision of FUT and Nano-fat Stem Cells Transplant
procedure at our clinic. During the Surgical evaluation, I noted certain hematological concerns that merit
further investigation and management by a hematologist. These concerns include Abnormal Platelet
counts & bleeding tendencies. It is essential to address these issues to ensure the safety and success of
any future medical interventions.
