    Begin collecting your documents of


    immunization records by Semester 1
    I.e.
    TB Skin test (TST) (Mantoux Test)
    Measles, Mumps and Rubella (MMR) immunity status
    Varicella immunity status
    Tetanus, Diphtheria and Pertussis (Tdap) immunity
    status
    COVID-19 Vaccine status
    Hepatitis B immunity status

    *Requirements may vary by institution
