1. Explain the value of a child centred model of assessment

I work for discovery home and one of the major works. The need of the child at the centre and
focus of the practitioners, the practitioner must observe the ongoing situation of the child and
bring to attention of the team so they can discuss and plan for their practical need of the child.
Our children have a typical or delay development due to their learning disability but should
develop the external exhibit a lot as individual example: some children develop at different
rates significant in one are or several areas. The practitioner should carry out assessment
and underline the main cause for their delay or atypical development. The practitioner should
support the child develop their plan on likes/dislikes. They should get more to stimulate their
children in this direction by motivating them, giving them extra care and extra option, for
example if a is sitting down and getting bored of watching a TV programme then as support
worker I would different activity i might do art craft or take them to computer room and play
game.
As a practitioner I would set them individual goal to make them independent. I would do
certain targets according to their special needs. For example if a child is walker and toilet
trained then i would think of different target they are
 Involve them to set up dinner table with folk and knife n plate.
 Involve them into more with washing up their own plates after dinner.

When a young person comes into respite care they are given an initial health assessment by
the care practitioner and other key workers and social worker, professional and the family.
they will know the specific needs and circumstances. It will be at this stage that we cant
continue with ongoing or day to day assessment, taking into account past medical histories,
that decisions will be made with regards of how best to meet the young person’s required
needs. The needs of a young person will come in categories of health, education and
domestic welfare.
All young people are different and their needs will be met as best to help them progress in a
positive way in all aspects of their lives. Regular checks and assessments will be done to
ensure that the subjects that have been put in place to help the young person are working to
ensure that the young person is getting the best care. Regular professionals meetings will
take place so that discussions can take place so that the best communication can happen to
ensure the best possible outcomes for the young people.

3. Explain the importance of working with others to assess the needs of children and young
people to inform planning (1.3 )

It is very important to work with others to assess the needs of children and young people,
especially our service users who have varied special needs and disabilities. This will enable the
various practitioners and professional and related agencies to develop a fuller, more accurate
picture of the needs of the child and therefore plan more effectively for all aspects of their
development. As Project workers, we work with ‘others’ including parents, careers, foster
careers, residential workers, social workers, psychologists, doctors, support workers, police,
youth justice, occupational therapists, speech and language therapists
 Parents- our setting work with parents to give them break so that they can relax
sometimes over the weekend. Parents request us for any specific placement.
 Carers- our setting works along the carer site t
 Project workers- Discovery home works with young person to support them with their
personal needs and to give them the best valuable time to make them feel comfortable.
 Psychologists, - our setting work with reports assessed and published by Clinical
psychologists of some of the children in our setting. The Clinical psychologists aim to
reduce psychological distress and enhance and promote psychological well-being of our
children with special needs and disabilities to help deal with their mental or physical
health problems which might include: anxiety and depression serious and enduring
mental illness adjustment to physical illness neurological disorders addictive behaviors
childhood behaviour disorders personal and family relationships doctors,
 police,
 youth justice,
 occupational therapists, - day care centre officials
 therapeutic workers such as those in bowling and swimming facilities
 speech and language therapists

4. Analyse current evidence about effective methods of ensuring participation and engagement
of children and young people of all ages and levels of understanding in assessment and
planning (1.4)

The effective methods/ways to engage child could be an appropriate venue or location. We


give our contributions through play, take their pictures for independent skills. We encourage
and allow the children to get engaged in setting the ground. We allow them to choice their
activity by giving them

5. Explain the importance of permanency planning for children and young people (AC 1.5 )

The importance of permanency planning is necessary for looked after children (in our respite)
who need to have plans made for their long term future. It’s to give each child or young
person greater sense of security and if possible a family for life. It takes into account the child
or young person’s history and needs, and how best to meet those needs in the future. It takes
into account individual circumstances and the age, needs and abilities of the child. It should
include the child and relevant others in decision making.

Our setting is a respite care where children are brought from their homes to spend some
quality time with us as their parents and carers take a break. It is our duty to ensure that they
feel at home as much as possible. This is achieved through permanency planning, where we
follow the care plans to provide the right service and help meet their holistic needs.

6. Explain how the goals and targets identified will support the achievement of positive
outcomes (AC. 2.3)
The goals and targets will be used with the children or young people in care setting, goals
and targets referred to regularly, progress will be monitored, achievement and success
would be recognised and celebrated, goals and targets reviewed as necessary.

Develop a plan with a child or young person and others to meet their needs with a focus
on the achievement of positive outcomes:

In our setting we set a three targets for each individual child goals and targets need to be
realistic and appropriate.

As practitioner we have to make good judgments of the abilities of children other than
reflecting on their ages and stage of development. This is because our children have
autism and delayed development and can not be expected to do things according to their
age but what they are able to achieve in their day to day care needs to develop
independent skills

We therefore take into serious considerations what they can really do into account
resources available and realistic timescales;

7. Explain how to support a child or young person and others to understand and agree the
goals, targets and outcomes of the plan (AC. 2.5)

In our respite care young person and others children are provided with clear information about
the roles and responsibilities of all those involved in the plan: e.g. clear communication and
instructions, using language that is appropriate to age and stage of development of child or
young person, use of one-to-one meetings or group sessions.
