小一入學申請備忘(2021 年 9 月入學適用)

(一) 派發申請表 ： 二零二一年六月二日至六月十日

日期 請於學校網頁下載表格。

(二) 遞交申請 ： 二零二一年六月二日至六月十日(恕不接受逾期申請)

(三) 遞交方法及 ： 用掛號形式於二零二一年六月十日或之前郵寄至本校：

(四) 遞 交 文 件 ： 1. 已填妥學生資料的「小一入學申請表」
2. 申請人出生證明書或身份證明文件副本；若申請人並無香港出生證明
書，家長須遞交申請人的外地出生證明書及獲在本港居留的身份證明
文件副本。
3. 教育局自行分配學位小一入學申請表副本
4. 小一註冊證副本/直資或私立小學註冊證明文件
5. 小一學位分配選擇學校表格副本(如有)
6. 申請人最近兩年的幼稚園成績表副本
7. 校外比賽/活動的經驗及成就證明文件副本 (不多於 4 張 A4 紙，可雙
面印刷)
8. 推薦信(如有)
9. 家長已回答的入學申請問題(按此下載題目)
10. 分別貼有$2.00 及$7.30 本地郵票及封面寫有學生姓名和回郵地址的信
封（約 9.5 吋 x4.5 吋）各一個，回郵信封用作寄回相關的收據及已遞
交的 USB。
11. 遞交一段申請人自我介紹及朗讀指定文章 (英語及普通話) 的錄
像片段，並把片段檔案儲存在 USB 記憶體內，片段要求如下：
錄像片段內容 時間
1. 用廣東話自我介紹 (內容自定) 1 分鐘內
2. 用英語朗讀文章 (按此下載文章) 2 分鐘內
3. 用普通話朗讀文章 (按此下載文章) 2 分鐘內

其他要求：
1. 以上內容的錄像片段可分開 3 個檔案存檔，並以考生姓名及語
言為檔案名稱(例如：陳小明廣東話)，以 MP4 格式存於 USB
記憶體內。
2. 錄像片段總時間不超過 5 分鐘。
3. USB 記憶體上貼有申請人的中文姓名。
(五) 費 用 ： 1. 交申請表時無須遞交任何費用。
2. 如獲入選，會安排學生進行視像面試，屆時才須遞交報名費用$40。

(六) 注 意 事 項 ： 獲面試的申請人會於 6 月 30 日或 7 月 2 日期間接獲本校電話通知。

申請人如於 7 月 2 日下午 5:00 後仍未收到本校電話通知，即表示落選，
家長無須致電學校查詢。

(七) 備 註 ： 1. 本校不接受小一重讀申請。
2. 申請者所有遞交的文件副本只作為是次入學試參考之用，本校會於公
佈結果後將有關文件全部銷毀，恕不退回。
3. 已遞交的 USB 會於公佈結果後寄回申請人。

Guidance Notes
(I) Distribution of Application Forms:

Application forms can be downloaded from the school website.

(II) Date of Submission:

2nd June 2021 – 10th June 2021 (Late submissions will not be accepted.)

(III) Submission Method:

(The date on the postmark should not be later than 10 June 2021). Please mark ‘P1 Admission’
on the front of the envelope. (Parents should not submit application documents to the school in
person.)

(IV) Documents to be Submitted:

a. Completed application form.
b. A copy of the applicant’s birth certificate or other identity supporting documents. (If the
applicant does not have a Hong Kong birth certificate, the parent / guardian should submit a
copy of the applicant’s birth certificate and a copy of the applicant’s valid travel document
or the resident permit.)
c. A copy of the Education Bureau’s Application Form for Admission to Primary One
(yellow).
d. A copy of the Primary One Registration Form / Registration Documents issued by a DSS
school or private school.
e. A copy of the Choice of Schools Form (if any).
f. Copies of the kindergarten school reports from the last two years.
g. Copies of certificates or documents of experiences and achievements in competitions /
activities (should not consist of more than 4 sheets of A4 paper; double-sided printing is
allowed).
h. Letter of recommendation (if any).
i. Completed admissions application questions (click here to download the questions)
j. Two self-addressed (9.5-inches x 4.5-inches) stamped envelopes (local $2.0 and $7.3)
showing the name of the applicant. The self-addressed envelopes are used to send the
application receipts and return the USB that has been submitted.
k. A video clip of the applicant's  self-introduction and  reading of the designated articles
(Putonghua and English) on a USB drive. The requirements for the video clip are as
follows:

1. Introduce yourself in Cantonese (customized content) Within 1 minute
2. Read aloud the article in Putonghua (click here to download the Within 2 minutes
articles)
3. Read aloud the article in English (click here to download articles) Within 2 minutes

Other requirements:
l. The video clip should include the above three items and be saved on a USB drive in MP4
format. Please name each clip with the applicant’s name and the language used in the
clip (e.g. PeterChanCantonese).
2. The video clip should not exceed 5 minutes
3. The applicant's Chinese name should be marked on the USB.

(V) Application Fee:

a. No fee will be charged for submitting the application form.
b. An application fee $40 will be charged for the shortlisted applicants who are invited to take
the entrance interview.

(VI) Important Notes:

Applicants who are invited for an interview will be notified by phone between 30th June and 2nd
July, 2021. Applicants who do not receive a phone call by 5:00 p.m. on 2nd July, 2021 may
consider his/ her application unsuccessful. Parents are not required to call the school for
enquiries.

(VII) Remark:
a. Applications from Primary 1 repeaters’ will not be considered.
b. All submitted documents are used for reference in the application only. The school will
destroy all submitted documents after the release of the application result, and the
documents will not be returned.
c. The USB will be returned to the applicant after the release of the application result.
