History of Present Illness: January 18, 2021 was admitted for management of anxiety. He was a regular in a
group therapy which lasted for 20 weeks, but moderately detached participant. He
was discharged, and was observed to be passive by delaying most of his tasks, not
paying his bills, and avoided contact with other people.
June 18, 2021 he went to the clinic accompanied by his sister for complaints of
loneliness, low self-esteem, feeling of void or nothingness, with feelings of social
inadequacy, suicidal ideation and discontent with anything. These complaints had
been chronic in nature, since childhood years. David remembered having daily
suicidal thoughts for several years in his early adolescent years. The sister noted that
his avoidant behavior was more notable than his level of anxiety when they attended
a family gathering. His most distinguished feature was a pervasive fear of being
mocked when interacting with other family members. Hence this admission.
