11/03/2020. No injuries reported and staff member has since been terminated.

Reporter is administrator at Cornerstone Cottage.

Complaint Summary: Rp reports that (child) was escalating in behavior and having an issue with a staff member on 11/03/2020. Rp reports
that the staff member struck . Rp reports that was slapped to the right side of the face/head by the staff member.
Rp reports that there were no injuries to . Rp reports that other staff were present and no physical harm happened. Rp
reports that the staff member was sent home immediately and has since been terminated. Rp reports that this incident took
place on Tuesday evening at 6pm. Rp reports that the staff member initially stated that she did not hit and was only
slapping hand away. Rp reports that video of the incident was reviewed Wednesday evening and it was determined
the staff member struck . Rp reports that case worker has been notified.
