SPORTS WAIVER

I, ____________________________________ parent/guardian of
__________________________________, a student of Eulogio “Amang Rodriguez Institute
and Science and Technology and a member of EARIST College Y Club, hereby acknowledge
and understand the following:

By my signature below, I acknowledge that I am aware of, appreciate the character of,
and voluntarily assume the risks of injury, including, but not limited to, bodily injury,
strains, and other ailments that could cause serious disability or death involved in
participating in all activities associated with the ECYC Sports TryOut Session.

By my signature below, on behalf of myself, my heirs, successors in interest, personal

representatives, and assigns, I hereby,

1. Waive any claim or cause of action against and release from liability the EARIST
College Y Club, its adviser, and coach for any liability for injuries to my person or property
resulting from my participation in sports tryout in EARIST Main Quadrangle in the activity
listed above;

2. Agree to indemnify and hold harmless the EARIST College Y Club, EARIST Manila
, its officers, employees, and agents for any claims, causes of action, or liability to any
other person arising from my participation in the said activity listed above;

3. Acknowledge that a participant and student of Eulogio “ Amang” Rodriguez Institute of

Science and Technology Manila, signature is required by the parent or legal guardian.

I HAVE READ THIS ASSUMPTION OF RISK, WAIVER OF LIABILITY AND RELEASE

AGREEMENT. FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN
UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND
VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING
MADE TO ME AND INTEND MY SIGNATURE TO BE A COMPLETE AND
UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED
BY LAW.
