COMMISSION ON AUDIT CIRCULAR NO.

96-006

TO : Heads of Departments, Agencies, Bureaus and Offices of the


National Government, Government Owned and/or Controlled
Corporations, Local Government Units; COA Directors; Chief
Accountants/Heads of Accounting Units; Heads of Auditing
Units; and All Others Concerned.

SUBJECT : Restatement with amendments of COA Circular No. 92-374


dated March 5, 1992 Re: Adoption of Forms and Guidelines for
the Quarterly Reports to be submitted by Agencies pursuant to
Section 82 of the general provisions, Republic Act No. 8174
(General Appropriations Act of 1996)

1.0 PURPOSE

This circular is issued to:

1.1 Revise the format and guidelines of the quarterly reports submitted
by the national government agencies (NGAs), local government
units (LGUs) and government-owned and/or controlled corporations
(GOCCs), to conform with the provisions of National Budget Circular
No. 440 re: Simplified Fund Release System, and
1.2 Provide additional details of expenditures in response to request of
Congress.

2.0 GUIDELINES

2.1 The head of each department, bureau, office or agency of the


National Government, including GOCCs and LGUs shall submit the
required quarterly reports listed in Section 3.0 hereof within 30 days
after the end of each quarter.
2.2 The quarterly reports shall show the financial data by province and
city.
2.3 In the case of GOCCs, appropriations and allotments under the
Special Accounts in the General Fund (Fund 151) and other funds
released by the National Government and the corresponding
obligations incurred, obligations liquidated, unobligated allotment
and results of expended appropriations shall be reported using the
prescribed formats.
2.4 The LGUs shall submit to the administering agency, copy furnished
the Accountancy Office, COA, a report on the Allotments,
Expenditures/Obligations Incurred and Disbursements/Obligations
Liquidated out of funds received from the National Government,
such as the Internal Revenue Allotment, Countrywide Development
Fund, Special Shares of LGUs in the Proceeds of National Taxes,
Municipal Dev't. Fund, Calamity Fund, etc., using the prescribed
format (Annex D). The administering agency is the NGA which
released the fund/funding check to the LGU concerned.
2.5 The administering agency shall include the reports (Annex D)
submitted by LGUs in their preparation and submission of Annexes
A and B.
2.6 The Chief Accountant shall at all times ensure that amounts
reflected in the quarterly reports tally with the figures in the Quarterly
Trial Balances.

3.0 REPORT FORMAT

The required reports (Annexes A to D) to be submitted are as


follows:

3.1 Statement of Cumulative Allotments, Obligations Incurred and


Unobligated Balances (Annex A)
3.2 Detailed Statement of Cumulative Expenditures/Obligations
Incurred, Obligations Liquidated/Disbursements and Unliquidated
Obligations (Annex B).
3.3 Report on the Results of Expended Appropriations (Annex C).
3.4 Statement of Allotments, Expenditures/Obligations Incurred and
Disbursement/Obligations Liquidated of the Local Government Units
(Assistance from the National Government). (Annex D)

4.0 DISTRIBUTION LIST

4.1 The reports (Annexes A to C) shall be prepared in five copies and


distributed to the following:
1) Senate Committee on Finance
2) House Committee on Appropriations
3) Accountancy Office, Commission on Audit
4) Department of Budget and Management
5) File
4.2 Annex D shall be prepared in three copies and distributed to the
following:
1) Administering Agency
2) Accountancy Office, COA
3) File

5.0 PENALTY CLAUSE

Failure of the officials/employees concerned to comply with the


requirements of this Circular shall subject them to penalty imposed under
Section 55, Chapter 10, Title I.B, Book V, Executive Order No. 292, the
Administrative Code of 1987 and Section 122 (2), Chapter 3, Title III, PD
1445, the State Audit Code of the Philippines.

6.0 SAVINGS CLAUSE

Cases not covered by this Circular shall be referred to the


Accountancy Office, this Commission for proper resolution.

7.0 REPEALING CLAUSE

Provisions of COA Circular No. 92-374 dated March 5, 1992 and


other existing rules and regulations inconsistent herewith are hereby
amended or repealed accordingly.

8.0 EFFECTIVITY

This circular shall take effect on January 1, 1996
