On February 1, 2024 you accepted the offer of employment with a start date on February 6, 2024 . With this, you agreed to
the terms and conditions of your employment contract and is fully aware that your employment is subject to the condition
of your full compliance with the Pre-Employment Requirements provision under Sec. 3, Par 1. You further acknowledged the
receipt of the Pre-Employment Requirements Checklist that was issued to you on even date.
Despite these reminders, you failed to submit your lacking pre-employment documents and comply with this mandate. Thus,
you are hereby instructed to submit the following pre-employment documents and requirements on or before
February 2, 2024.

1. MEDICAL

Your failure and/or refusal to strictly comply with this notice may subject you to progressive disciplinary action/s,
up to and including termination of employment. Further, please be reminded that your failure to provide these
mandatory government documents may result to the delay or none remittance of your government mandated
contributions, as well as your salary for the affected pay period.
We advised that you take this matter seriously and give your much-needed attention. For any inquiries please get in touch
with the undersigned or your Employee Experience Business Partner.

PRE-EMPLOYMENT REQUIREMENTS
FOLLOW-UP CHECKLIST
As provided by your employment contract, the terms and conditions of your employment is
subject to the condition of your full compliance with the Pre-Employment Requirements provision
which state as follows:

“Section 3 (b)(i)

You hereby acknowledge that full and satisfactory compliance with your pre-
employment requirements is also an indispensable condition for continued
employment. You, therefore, acknowledge the Company’s right to rescind
your employment contract and/or to terminate your probationary
employment if:

i. You fail to submit all pre-employment documents and requirements within


five (5) days from your start date.”

Know that management values your employment with the Company and is committed to provide
you with the best employee experience there is, hence, we request for your utmost cooperation.
Let this serve as a reminder that you are required to submit all your lacking pre-employment
documents within five (5) calendar days from your start date.
Please note that your failure to submit the above requirements may cause delay in the
processing of your government related contributions/taxes and the Company may not be able
to process your pay for the affected pay period.
