Analyzing Health Habits

INSTRUCTIONS

Use this worksheet to review your current lifestyle habits and plan any changes you’d like to make.
When you feel physically healthy, it’s easier to do your best work.

If you have changes you want to make, aim for specific actions that will be easy to implement and
that you can sustain.

1. Exercise releases stress, improves mood, and sharpens focus. If you feel you need more exercise, are
there any ways you could change your daily routine so that you’re moving more and sitting less?

2. Is there any kind of exercise you’d like to try—or add to your existing habits—such as stretching or
strength-training?

3. Are you satisfied with your eating habits? What’s working well, and what’s not?

4. Is there anything you’d like to change about what you eat? Some ideas include but aren’t limited to: eating
fewer takeout meals at lunch, or reading food labels to find healthiest version of a product you regularly
consume.

5. Is there anything you’d like to change about how you eat? For instance, do you enjoy your food or eat
quickly while trying to get other things done.

6. Do you get restful sleep most nights? If not, what affects your sleep?

7. List one thing you’d like to try to get more restful sleep. For instance, leaving your cellphone in another
room, so you’re not awakened by alerts.
