Name of the client : Mr. Smith

Location : Mr. Smith's room

Mr. Smith was found lying on the floor by his bed the morning after
handover. David alerted the RN, who assessed Mr. Smith and found bruising
on his right elbow but there was no obvious injury. The RN instructed David
to perform half-hourly neurological observations on Mr. Smith for the next
four hours. Arun assisted in obtaining the hoist machine and together to use
the hoist to lift Mr. Smith back into bed.

Smith’s Condition
 Mr. Smith appeared asking inappropriate questions and confused.

Actions taken
 David ensured Mr. Smith's comfort and safety throughout the process.
 Mr. Smith was reassured by David and reminded of his situation.
 David reported the incident to the RN, who provided instructions for
neurological observations.
 The hoist machine was used to lift Mr. Smith, minimizing manual
handling risk.
 Continued monitoring Mr. Smith's neurological condition as per nurse's
instructions.
 Ensured Mr. Smith's well-being and comfort.
