1. Brief summary of family background indicating the following:

a) When and where did husband and wife met?

b) Date of wedding.

c) Number of common children (Please indicate respective names and ages)

d) Incidents of violence/maltreatment and/or any other circumstances promting the filing of

legal separation.

e) Date since when husband and wife separated.

f) Other narration of facts constituting legal separation.

2. Copy of identification cards husband and wife indicating present address.

3. Original copy marriage certificate

4. Original PSA copy certificate of live birth of common children (if there is any)

5. List of properties

6. List of creditors

7. Affidavit of witnesses
