Please note you MUST arrive 15 minutes before the start of your test. Test times CANNOT be changed unless FIVE working days’ notice is given. Failure to arrive
on time will result in you failing the test and being changed in Full. Free car parking available on site.

If you have taken your Health & Safety Touch Screen Test in the two weeks prior to your test please bring your pass letter with you.

If your test includes an NVQ Assessment and you have not previously submitted your Witness Testimony then please bring this with you.

ALL candidates MUST provide photo I.D. upon request in the form of a Passport or Driving Licence (Site Cards are Not accepted). MUST BE VALID AND IN DATE
Failure to show ID can lead to being turned away from the test.

With the exception of A61 – Appointed Person – Lifting Operations, delegate should provide their own personal protective clothing in a clean working order,
which includes safety footwear, hard hats, gloves, Hi-Viz waistcoat etc., for all training/assessments.
TERMS AND CONDITIONS
Reservations
Initial reservations can be made over the telephone. An initial booking confirmation will be sent via email. Full joining instructions will be forwarded following
receipt of a non-refundable deposit payment. If joining instructions have not been received within 24 hours of leaving payment details, please telephone
01543899706 to make sure payment has been received. Course dates will not be held until a deposit payment has been left.

Fees
Unless otherwise specified in the quotation or other agreement, invoices for goods or services are for payment within 30 days from the date of invoice. For Clients
who do not hold a credit account with MPTT, payment must be made in full prior to commencement of training. If full payment has not been received, for Client's
not holding a credit account, MPTT reserves the right to refuse delegates entry to the course and charge in full as per the cancellation terms.
All fees quoted are exclusive of VAT, which must then be paid in addition to the quoted fee at the time of payment. VAT is charged at the current rate of
Government legislation.
Fees will be correctly quoted at the time of booking, and initial quotes will be honoured. However, until confirmation is completed MPTT reserve the right to alter
fees at any time.

Venue/Accommodation
Details of how to get to the venue will be included in the joining instructions. MPTT is not responsible for providing information or the booking of
accommodation, and any queries of this nature must be handled by the hotel directly. The course fees do not include any accommodation or entertainment costs.

Course Literature
All required course notes are included in the course fees. Course literature is copyright and may not be reproduced without permission.

Cancellations and postponement


If less than two weeks’ notice is given, employers/delegates are responsible for the full cancellation charge.
Cancellations can be made by phone, but must be confirmed in writing, either via email or post. If written confirmation is not received, the cancellation will not be
processed and the delegate will be liable for the full cancellation charge.
Failure of any element of a course or test that is a prerequisite for subsequent elements will result in part or all of the booking being charged in full. Additionally
failure to supply any prerequisite documents or comply with other requirements prior to the commencement of the course or test will result in part or all of the
booking being cancelled and charged in full.
If MPTT cancels a course delegates will be informed and an alternative date will be offered, however, no refunds will be offered. MPTT will not be held liable for
any costs incurred by The Client as a result of a booking cancellation or contracts for reasons beyond MPTT’s control.
Events being delivered at a site specified by The Client must be suitable for the requirements of the event and meet any relevant approved codes of practice as set
out by MPTT or any related awarding body. Failure in the aspect of site suitability above may result in MPTT cancelling the booking without notice.
Any delegate who fails to attend, is excluded from or refused entry to, any part of a booking or test (in accordance with the terms above) will be charged at 100%
of the booking or contract price.
If it is believed that any delegate is under the influence of alcohol or illegal substances MPTT reserve the right to refuse training, testing and assessment. The
delegate/employer will be liable for the full cost of the course.

Transfers
It is possible to transfer a training course to another individual, providing all course prerequisites are met. Requests must be made in writing, and the
delegate/employer is liable for any additional costs this may incur. MPTT reserve the right to refuse candidate transfers, where prerequisites aren’t met, and the
full course cost payable by the delegate/employer.

Data Protection and Privacy


Any information disclosed to us either directly or through MPTT’s website, by whatever means will only be collected, stored and processed in accordance with the
requirements of the business between MPTT and The Client and as set out in our Privacy Policy.

NVQ enrolments
NVQ’s assessed on site will be invoiced over two part payments. In order to process an enrolment 50% of the full NVQ fee is required, the remaining 50% will
then be invoiced on completion.
Once your initial payment has been received we deem this as your commitment to the full programme therefore Midland Plant will process all fees required for
registration and assessments via the awarding body therefore no refund will be available for those who do not wish to continue or do not complete. Clients will
also remain liable to pay the remaining 50% NVQ fee in these instances.
Midland Plant will allow your registration and enrolment to remain live for 2 years. If your assessment does go past this period we will assume you do not wish to
continue therefore your registration will be removed from the centre. If you wish to continue at a later date, additional fees may apply.
It is the candidates responsibility to maintain contact with their assessor throughout the assessment, however if you have any issues contacting your assessor or
other issues that may cause a delay in you progressing please advise the office at your earliest opportunity so we are able to offer support.
Midland Plant cannot be responsible for other licence cards expiring during your NVQ enrolment period preventing upgrades, this is the responsibility of the
candidate. We are however always happy to assist and offer advice to prevent this where possible.
Certification and licence cards will not be processed until we are in receipt of full payment
