    I, the undersigned, willfully and voluntarily give consent on the participation of my child
    __________________________of Grade 12- ___________ in the “PROJECT INDAK (Indayog at
    Kultura) 2024 ,as a performance output in Health Optimizing Physical Education (HOPE) 3 to
    be held on February 5, 2024 at NNHS Cong. Sal Fortuno Civic Center from 7:00-12:00 NN.

    I have considered the benefits that my child will obtain from his/her participation
    provided that due care and precautions will be observed to ensure the comfort and safety of
    my child and that DepEd employees and personnel may not be held responsible for any
    untoward incident that may happen beyond their control.
