we have authorized Ms.Tara Doherty of Lucaya InternaTonal School Limited (LIS) to take Our Child on a school trip
to parTcipate in the class trip to:

A classroom without walls. Lunch on the beach, Learning for Life followed by PE.

We agree that Our Child will abide by the LIS Code of Conduct and the Event rules and realize any breach of these
rules may result in suspension or dismissal without the refund of any fees.

I authorize LIS through its authorized representaTve(s) named above, to take whatever acTons it may consider
warranted under the circumstance regarding my child’s health and safety. I fully release LIS and its authorized
representaTve(s) from any liability for such circumstance or acTon as may be taken in connecTon therewith. I
authorize LIS, or its authorized representaTve(s), at its discreTon, to place my child, at my expense and without
further consent, in a hospital for medical services and treatment, or to place my child in the hands of a licensed
doctor for treatment. LIS may elect to access my family health/accident policy.
