Dear Parents,
This legal consent form must be signed and returned to school before your son/daughter will be allowed to
attend school on campus. Please print the consent form, fill in the details and email the scanned copy to the
school at the given email address: Kda.hybrid.learning@bayviewhigh.edu.pk or submit a hard copy at the school
gate by Wednesday, 3rd February 2021 between 09:00 am till 02:00 pm.

Hereby give consent for our son/daughter to attend BVHS for physical face to face instruction on the school premises
from February 2021 onwards.

We understand that whereas Bay View High School will follow the standard operating procedures recommended by
the government with respect to the health and safety of our child while they are on the school premises, the school
cannot be held liable for any health issue arising after the child has rejoined the school campus and that our child will
attend school entirely at our/his/her own risk.

Each student will be required to follow the school SOP’s. These will include: 1)
Washing hands regularly,
2) Using hand sanitizer,
3) Wearing a mask at all times,
4) Following seating / desk arrangements and
5) Following protocols while moving from one area to another on the campus premises, as per school directions.

We also undertake to follow social distancing at all times in our personal lives/capacity including the Government
and School SOPs for both parents and our child, to take every precaution to avoid the possibility of our child
contracting COVID 19 or being the source of the spread of the infection in the school, for the good health of our own
child and the greater good of the other students, staff and the entire school community.

We undertake that we will inform the school promptly in case we/our child / anyone within the household including
help, tests positive for COVID 19 or exhibits symptoms associated with COVID 19.

In such a case, we undertake to comply with quarantine requirements and also to abide by any request / direction
issued by the school to prevent the child from attending school for such period as may be determined by the school
management. We further undertake to comply with any request made by the school management in such
circumstances or even otherwise to test our child by PCR test for COVID 19.

We also undertake that if we plan to travel out of Karachi, either domestic or international, we will inform the school.
We will also have our child tested through a PCR test and send in a negative test result before they will be allowed to
re-join school.
