by the Govt. of India and Govt of MP school Education department. CARMEL CONVENT SR. SEC. SCHOOL,
NEEMUCH will not be responsible if my daughter/ward contaminate any COVID – 19 symptoms after
attending the regular classes. I do hereby confirm that my daughter/ward will compulsorily wear face
masks within the School campus and follow all the COVID – 19 protocols such as washing hands, using
sanitizers and maintaining social distancing etc.

 CONSENT FORM FOR SENDING MY WARD
TO Carmel Convent Sr. Sec. School, Neemuch

As a parent/guardian of Student of Carmel Convent Sr. Sec. School, Neemuch, I hereby give my consent
for my ward to attend the School and agree to the following points:
• I understand that my ward will start attending school offline as per the timetable.
• I understand that My ward will only be allowed to School with this consent letter.
• By submitting the Consent Form, I agree to sending my ward to school voluntarily and on my
own will.
• I understand that the school will follow the standard operating procedure (SOPs) regarding
health and safety precautions for COVID19 issued by the Ministry of Health, Govt. of India, and
State Government. In case of any symptoms of COVID19 surfacing in my ward, I shall take
him/her away from the school immediately and I shall send him/her back again only when
he/she completely recovers from COVID19.
• I understand that COVID-19 is extremely contagious and is believed to spread mainly from
person-to person contact.
• I understand and accept the fact that I shall have to submit an RTPCR negative Covid test report
to the school when my ward joints back again.
• I understand that CARMEL CONVENT SR. SEC. SCHOOL, NEEMUCH has put in place preventative
measures to reduce the spread of COVID-19; however, CARMEL CONVENT SR. SEC. SCHOOL,
NEEMUCH cannot guarantee that a student or their family or friends or to anyone with whom
such persons may have contact, will not become infected with COVID-19.
• I shall abide by all the Government rules and SOPs of Covid management in school and at home.
• My ward will have to carry every class day to school a mask (with one extra) plus 70% alcohol
based sanitizer.
• I also understand that each class day there will be temperature check at the school gate and any
student with temperature of more than 990 F, will be sent back or made to sit in the isolation
room and parents would be informed to come and take her home.
• I understand and acknowledge that given the unknown nature of COVID19, it is not possible to
fully list each and every individual risk of contracting COVID19. Hence, I shall not hold the school
responsible under any circumstance in case my ward gets infected by it in future.
