hereby undertake the following:

1. I have carefully read all the guidelines/advisory issued by the central and state government,
district administration and NLIU Bhopal regarding my stay/visit during the current COVID-
19 crisis. I am also aware of the UGC Guidelines on COVID-19 crisis and will comply with
all the given guidelines.
2. I am voluntarily willing to return to campus and will strictly follow all the guidelines laid
down by NLIU Bhopal regarding COVID-19 pandemic.
3. I confirm that I will be arriving/ have arrived from__________________________(place)
and this place is not a containment zone for COVID-19.
4. I confirm that I have tested myself for COVID-19 on _____________________ (date) and
have tested negative. I am attaching a copy of the report.
5. I confirm that none of my family members have tested positive for COVID-19 and that I
have not been in physical contact with any COVID-19 infected person during the previous
14 days.
6. I am in complete understanding of the current COVID-19 status in Bhopal, the risks
involved in travelling to the NLIU campus, and the COVID-19 risk involved during my stay
on campus. NLIU shall not be deemed responsible if I contract COVID-19 either during my
travel or stay on NLIU campus.
7. In event that I violate any of the guidelines issued by NLIU and/or guidelines issued by the
central and state government and district administration, I shall be subject to such action as
prescribed by the NLIU guidelines in addition to such action as may be specified in the
guidelines issued by the governments from time-to-time.
8. Students once entered will not be allowed to go outside the campus. However, in exceptional
circumstances like medical urgency et appropriate authority will permit the student to leave
the campus. Any violation of the guidelines, students may not be allowed to stay in the
campus till the situation improves.

Ms. do hereby undertake and confirm that I am willingly
allowing him/her to travel to the NLIU Bhopal campus. I declare that in any unfortunate
situation, if he/she gets affected with COVID-19 infection, while returning to the camps or
during his/her stay in the campus, the Institute will not be held responsible for it. Furthermore,
I also undertake to bear all the medical expenses in case of infection of the student. I am
willingly taking the risk of sending my child to the NLIU Bhopal campus.
