Sub: Permission/Consent for attending the school

Dear Sir,
The school will be reopened from 14.12.2020 as per the orders of Educational Department/State Govt.
with the following various precautions to contain Pandemic Corona and SOPs as issued by the
Ministry of Health and Family Welfare, MHRD/MHA/ other departments. We seek your consent, if
you feel that your ward must attend the classes.

I father / mother of of

Class Section hereby extend my consent to my ward to attend the school

on _________/ or from __________ to ___________. I assure that my ward is completely

fit with no symptoms of any sickness. I will manage the pick and drop of my ward.

I assure you that apart from the precautionary / safety measures made by the school, I will make sure
that my ward shall also follow the precautions such as :-
1. Using of Face mask/shield.
2. Carrying Alcohol based Hand sanitizer, liquid / paper soap.
3. Carrying own water bottle & food articles and not sharing with other fellow students.
4. Maintaining social/physical distancing strictly.
5. Occupying the seat allocated by the teacher.
6. Ensuring frequent hand-wash.
7. Following all the rules and regulations/guidelines/instructions of the school.

Further, I undertake that I am aware of pandemic Covid-19. My ward shall not attend school in case of
sickness/ill health and shall report about the indifferent health of any family member in the house
immediately and shall not hold anyone responsible for my ward’s health.
