TO WHOM IT MAY CONCERN:

barangay for a period of 1 year subject for renewal, provided that the activity/ties inviolate sections or
provisions of the municipal or provincial ordinance regulating and governing thereto and agree with the
conditions set by the barangay council.

Whereas, any contravention of the above-mentioned related laws during the course of operation
shall annul the validity of this permit and be required to recompense the corresponding penalty as the law
prescribed.
