HYPERTENSION AND DIABETES CLUB OF BARANGAY CARIKKIKAN NORTE
REACTIVATION

II. INTRODUCTION:
To fight NCDs, it is important to practice healthy lifestyle choices early. Also, early
detection and care using cost-effective and sustainable health care interventions that are
integrated in primary health care facilities are essential.
The DOH Non-Communicable Disease Prevention and Control Center reported a total of
600,000 patients are registered nationwide in the HPN and DM Clubs as of February 2016.
To become a registered member, a patient must consult the nearest health center or
primary health care facility in the community for proper assessment, screening and management
using the Philippine Package of Essential NCD Intervention (Phil PEN) protocol. Once he/she is
diagnosed to have hypertension and/or diabetes, he/she will be enrolled to the club.
A registered member is entitled to free maintenance drugs for hypertension
(Losartan/Amlodipine/Metoprolol) and diabetes (Metformin).

III. OBJECTIVES:
The main objective of this practice is to provide free medical consultation, facilitate the
distribution of free maintenance medicines for High Blood Pressure, Cholesterol and Diabetes to
control and prevent the complications of these non-communicable diseases (NCDs) and above
all, to empower the Barangay Health Workers and members of the Club to address the needs of
the patients and take care of themselves respectively.

IV. TARGET PARTICIPANTS:

 Community folks with elevated blood sugar and are hypertensive.
 Barangay Officials
 Barangay Health Workers (BHW and BNS)
 City Midwife
 HRH deployed (Nurse)

V. METHODOLOGY:
 Information dissemination campaign
 Small group Discussion
 Re-election of participants

VI. RECOMMENDATION:
After years of continuous follow-up consultation and giving of free maintenance
medicines to patients enrolled in the Hypertension and Diabetes Club, most of them have
controlled blood pressure as well as blood sugar. Aside from these advancements, patients have
increased awareness and knowledge about their medical condition. There is a significant
improvement in the health-seeking behavior of every individual in the community as showed by
the increasing number of consultations in the healthcare facility, due to this we highly
recommend that there should be constant and regular supply of medication from DOH/ Local
Government unit to sustain this kind of results.
