SAFETY WORK PROCEDURE: Sewer & Manhole CCTV Inspection

Doc No: SWP-01

1.Purpose: To define a method and provide a comprehensive Safety Work Procedure (SWP) for
the Process of Manhole and Sewer CCTV inspection.

It sets to identify the hazards involved in performing each task and the inherent risks as
well as to establish guidelines for the associated control measures, including what
personal protective equipment is required, and the steps involved to complete each
activity without incident.

2. Scope: This procedure applies to all lines and Manholes where Closed Circuit Television
(CCTV) for inspection of Sewers, Drains and Manholes is utilized.

A CCTV inspection is designed to ascertain the condition of the water main to provide data assessing:
(1) The structural integrity of the pipeline system
(2) The service condition of the pipeline system
(3) The possible cause and effect of any extraneous water infiltration/ex-filtration to/ from the
pipeline system
(4) The location and extent of the expedient connections to the pipe line system.

3. Reference: QP-424-01 - Control of Records

4.3 Work Activity: Lowering Down CCTV Camera

The site supervisor must issue a Permit to Work at Heights before the activity is undertaken.
All workers must complete a “Working at Heights Course for Workers” and all Supervisors a “Working
at Heights Course for Supervisors”.

Safety Practices
 Fall prevention configurations are the preferred form of fall protection (a guardrail should be installed whenever it
is possible).
 Fall arrest configurations should only be used when fall prevention systems are not practical or as a back-up to
fall prevention.
 At least 2 workers must be present when a restraint or arrest system is used
 Keep your free fall distance to a minimum.
 Attach the lanyard directly overhead, where practicable.
 Ensure that a fall arrest system is attached to a suitable anchorage point.
 Take into consideration obstructions that are below the work area.
 Ensure that barricades, warning tape and signs identify restricted areas.
►Do not use defective equipment or components.
►Do not use a harness or lanyard that has arrested a fallen worker.
►Do not attach two lanyards together to make them longer

Reminders
Guardrail
 Ensure the guardrails are secured so they cannot be moved in any direction.
 If a portion of a guardrail has to be temporarily removed then the exposed workers must be protected by another
fall protection system.
Fall Arrest System
 Worker must determine and assemble the necessary components.
 Worker must calculate the total fall distance and ensure that an adequate clearance distance is available.
 Worker must ensure that the harness is adjusted to fit properly and is rated for his/her weight.
 A full body harness and appropriate lanyard must be secured to an anchorage point, lifeline or static line.
 Only one worker is permitted to be attached to a lifeline at any one time.
 A lifeline must be provided with softeners at all sharp edges or corners.
 A shock absorber should be used to reduce and control the maximum arrest force that occurs in a fall.
 A self-retracting device can only be used as a form of fall arrest. Worker must field test the device before using

 Falling from height  Tripod in place to lower down the camera
 Hands getting caught in  Worker connected to a lifeline properly
lifting cable anchorage
 Permit to Work at Heights

4.4 Work Activity: Manhole entry and exit

Working in manhole

Only in seldom situations is it necessary to actually enter the Manhole. When for some reason the
camera gets stuck in the process of lowering it down. In the event of this happening all safety procedures
related to working in Confined Space must be put in place (see detailed RA

Working in Confined Spaces can be hazardous so strict observance of all safety procedures is
paramount. Site Managers shall undertake a risk assessment and fully brief all team members to ensure
that they all know the tasks they are to perform.

o All supervisors must have an SOC in working in Manholes.

o All workers expected to enter the Manhole must have undertaken a SOC for working within
manholes.
o A Permit to Work in Manholes (Confined Spaces) by the Site Supervisor must be issued for each
Manhole (Confined Spaced) whose access is considered essential to carry out the CCTV works
and dully filled in and signed by all the men involved in the CCTV procedure (see “Confined Space
Entry Permit”);
o A certified Confined Space Assessor must inspect and ensure that the Manhole is in a safe
working condition:
o Toxic Substances:
• Methane (CH4): between LEL 53% and UEL 15%
• Hydrogen Sulphide (H2S): between LEL 4.3% and UEL 45.5%
• Carbon Monoxide: between LEL 12.5% and UEL 175%
o Oxygen deficient atmosphere: Minimum of 19.5% with a maximum of 23.5%

4.5 Work Activity: Sewer Cleaning by high pressure water jet

When water is pressurized to 4,000 PSI, it becomes a potentially deadly force that can easily result in
serious injury when the water jet comes into contact with skin or eyes. Awareness of common jetting
hazards, knowing how to protect oneself from them, and learning and practicing the proper safety
procedures can greatly reduce the chances of disaster striking at unexpected moments.

For this reason, it is our safety requisite that all operators attain a “Certificate of Proficiency in Sewer
and Manhole cleaning” recognized by PUB.

Sequence of activities & Best Practices:

1. Position the sewer jetter downstream
2. A screen (debris catcher) should be applied at the downstream sewer manhole channel to trap and later remove, all
grit, silt, sand, grease, roots and debris
3. Attach a nozzle to the sewer high pressure hose. Do not over tighten and never aim it towards anyone
4. Position the roller guide in the sewer
5. Operating the sewer jetter:
5.1. Make sure there is sufficient water in the water tank;
5.2. Insert the hose into the sewer through the hose guides;
5.3. Make sure the nozzle is inserted at least 1.5m into the sewer. Failure to do this can result in injury or death;
5.4. Before starting the engine, perform the safety check as recommended by the equipment manufacturer;
5.5. Start the engine and let the engine idle and warm up for a minute or two. In the meantime, water flows through
the hose and purge any air in the system;
5.6. Turn the water pressure regulating valve to the desired pressure;
5.7. Increase the engine RPM to the recommended speed;
5.8. Operate the hose reel lever to allow hose to travel into the sewer under the propulsion power of the high pressure
water flow. This step will also depend on the type of cleaning functions required;
5.9. Never withdraw operating nozzle out of the manhole. The high pressure water from the nozzle may cause
severe injuries!

4. Hand-gloves must be worn prior to lowering of camera;
5. Lower down CCTV camera into the Manhole;
6. During lowering an assistant will assist;
7. At all times monitor sewage level during survey;
8. If heavy rain should commence Stop Work Immediately;
9. Smoking is not allowed within 5m radius from Manhole opening;
10. Once the survey is completed, close all Manhole covers which are open;
11. Remove Cones and red and white tape/poles from site;
12. Clear all debris;
13. Ensure all Manholes are closed once more before leaving the site.

4.6.1 Hazard 4.6.3 Control Measures
