resident of Purok No. 3, Barangay SAN JOSE, Esperanz, Agusan del Sur, a law-abiding
citizen and has a good relationship with the community.

This further certifies that above-mentioned person belongs to the indigent

families identified by the barangay government considering that he/she has no
sufficient/permanent income or employment.
