Designing, Testing and Revising

Survey Questionnaires

Introduction

Getting along with surveys every day is as important as planning, making a

decision, learning other's behaviors, understanding trends, and assessing
situations. Gathering ideas and feedback may contribute to what we want to
develop, bring clarity to our aims, and reevaluate different aspects and options.

Learning Competency

Design, test and revise survey questionnaires

CS_EN11/12A-EAPPIIe-j-8

Objectives

At the end of the lesson, the learners should be able to:

1. define objectives, desired analyses, and target population

2. develop questions and construct questionnaires; and
3. test and revise questionnaires.

Discussion

Surveys are commonly mistaken as mere questionnaires. A survey is

a set of questions aimed at gathering specific data from a targeted group of
people. A survey is an essential tool in finding out how a particular group of
people think, feel and behave. Traditionally, it has the intention of
generalizing the processed information to represent the notion of the target.
The result may come from critical sources and regard as notions of
everyone. Standard procedures are applied to get reliable information.

Designing a survey questionnaire

Desinging a survey questionnaire involves certain steps that will help

a researcher create questions that are focused on extracting intended data
from the target population.

1. Establishing Goals- the following questions will help you analyze

your goal.

2| P a g e

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

Sample Statement
a. What do you want to I want to know what are the students’
learn or discover? preferences in different learning
modalities during the COVID19
pandemic.
b. What are your goals By identifying the percentage of
in conducting the students’ preferences in different
survey? learning modalities, it gives clear
insights on how teachers effectively
deliver quality education during the
pandemic crisis.

2. Selecting your samples

a. To whom will you Senior High School students of New

conduct the survey, and Taugtog National High School
what questions will you
be asking?
b. How many people are 35 grade 11 students
you planning to 35 grade 12 students
interview?

Remember that a small representative sample will reflect the

group from which it is drawn. The larger the sample the more
precisely it reflects the target group.

Looks Great! After designing your questionnaire, let’s

understand how to make your own questionnaires by learning the
different types of Survey questions and survey scale responses.

Developing Questionnaire
In developing your questionnaire, let us identify first the types of
survey questions and survey response scales

lOMoARcPSD|29291824

Types of Survey Questions

1. Closed – Ended Questions

Closed Ended Questions limit the answers of the
respondents to response options provided on the questionnaire.
It is time-efficient, responses are easy to analyze and ideal for
quantitative type of research.

Examples:

a. Dichotomous or two-point questions (YES or NO, Satisfied or

Unsatisfied)

Yes No

a. Do you like shopping?

b. Have you tried shopping online?

b. Multiple choice questions

c. Which of the following items you usually buy online?

___________ cosmetics
___________ gadgets
___________ Mobile Accessories
___________ Men’s & Women’s Apparel
___________ Men’s & Women’s Accessories
___________ Groceries
___________ Toys, games & collectibles
___________ shoes

4|P age

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

c. Scaled questions that make use of rating scales

such as the Likert Scale.

2. Open-Ended Questions
Open-Ended Questions have no pre-defined options or
categories. The participants disregard connection except
capitalization and punctuations. Participants respond to the
questions exactly as how they would like to answer them. The
researcher can investigate the meaning of the responses, ideal
for qualitative type of research. This type of questions are time
consuming and responses are difficult to interpret.

Examples:

a. Completely unstructured questions – openly ask the Opinion

or view of the respondent.

d. What are your recommendation/suggestions to

improve shopping services?
______________________________________________________

_______________________________________________________

5|Page

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

b. Word association questions – the participant states the first

word that pops in his mind once a series of words are
presented. It is a method in which respondent’s opinions
and perceptions are assessed.

Associate the following words:

1. Intelligence ___ Positive ___ Neutral __ Negative

2. butterfly ___ colorful ___ playful __ free

c. Thematic Apperception Test – a picture is presented to the

respondent which he explains on his own point-of-view

d. Sentence, story or picture completion – the respondent

continues an incomplete sentence or story, or writes on
empty conversation balloons in a picture.

Feel free to give your insights:

1. My online shopping experience is

__________________________________________________.
2. The quality of the product I bought was
___________________________________________________.

6|P age

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

3. Matrix questions
Matrix questions are also closed-ended questions but are
arranged one under the other, such that the questions form a
matrix or a table with identical response options placed on top.

Example:

Factors Strongly Satisfied Neutral Unsatisfied Strongly

Satisfied Unsatisfied
Smell
taste
texture
Overall
appearance

4. Contingency Questions
Continency questions are questions that need to be
answered only when the respondent provides a particular
response to a question prior to them are called contingency
questions. Asking these questions effectively avoids asking
people questions that are not applicable to them.

Example

Do you play Mobile Legends? ____Yes _____ No

If YES, how many hours do you spend in playing Mobile

Legends.
______ 1 hour
______ 2 hours
______ 3 hours
______ 4- 5 hours
______ more than 5 hours

Survey Response Scales

The response formats used in surveys vary depending on the

type of question being asked. Each of these response scales has its

7|Page

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

own advantages and disadvantages, but the rule of thumb is that the
best response scale to use is the one which can be easily understood
by respondents and interpreted by the researcher.

1. Dichotomous Scales
A dichotomous scale is a two-point scale which presents options
that are absolutely opposite each other. This type of response scale
does not give the respondent an opportunity to be neutral on his
answer in a question.

Examples:

Yes- No True - False

Fair – Unfair Agree – Disagree

2. Rating Scales
Three-point, five-point, and seven-point scales are all included
in the umbrella term “rating scale”. A rating scale provides more than
two options, in which the respondent can answer in neutrality over a
question being asked.

Examples:

2.1. Three-point Scales

Good - Fair – Poor
Agree – Undecided - Disagree
Extremely- Moderately - Not at all
Too much - About right - Too little

2.2. Five-point Scales (e.g. Likert Scale)

Strongly Agree – Agree – Undecided / Neutral - Disagree -
Strongly Disagree
Always – Often – Sometimes – Seldom – Never
Extremely – Very - Moderately – Slightly - Not at all
Excellent - Above Average – Average - Below Average - Very Poor

8|P age

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

2.3. Seven-point Scales

Exceptional – Excellent – Very Good – Good – Fair – Poor – Very
Poor
Very satisfied - Moderately satisfied - Slightly satisfied – Neutral
- Slightly dissatisfied - Moderately Dissatisfied- Very dissatisfied

3. Semantic Differential Scales

A semantic differential scale is only used in specialist
surveys in order to gather data and interpret it based on the
connotative meaning of the respondent’s answer. It uses a pair
of clearly opposite words, and can either be marked or
unmarked.

3.1 Marked Semantic Differential Scale

Please answer based on your opinion regarding the new
line of skin cream.

Very Slightly Neither Slightly Very

smooth rough
creamy lumpy
Light greasy
Rich dull
texture

3.2 Unmarked Semantic Differential Scale

The central line serves as the neutral point:

Smooth ___________________l__________________ rough

Creamy ___________________l__________________ lumpy
Light ___________________l__________________ greasy
Rich
Texture ___________________l__________________ dull

Here are the General Considerations:

1. The first rule is to design the questionnaire to fit the medium.
2. KISS - keep it short and simple.

9|Page

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

3. Start with an introduction or welcome message. A good

introduction or welcome message will encourage people to
complete your questionnaire. When practical, state
who you are and why you want the information in the survey.
4. Allow a “Don't Know” or “Not Applicable” response to all
questions, except to those in which you are certain that all
respondents will have a clear answer

How to write the Questions?

1. Focus On Your Survey Goals and Objectives.

Every question presented should be related to the goal of
the survey.

2. Identify the Attributes That Need to Be Measured.

Based on the survey goal, ask yourself what you want to
measure from the responses of the participants. These
attributes include the respondents’ demographics, knowledge,
skills, attitude, perceptions, behavior, intentions, beliefs and
goals.

3. Select the Appropriate Types of Questions.

Analyze and review what kind of question are you going to
use in a survey questionnaire.
4. Choose the Types of Response Options.
Understand the types of responses options.
5. Check the Questions for Reliability.
Reliability is an important measure on the consistency of
the survey results that are gathered by using the questionnaire.
It simply answers the question, “Do the respondents truly
understand the meaning of the questions as they are stated?".
The most commonly used reliability test for survey questions is
the test and re-test method that serves as the pilot survey.

Pre-testing (Pilot testing)

According to Hu S. (2014),’’Pretesting is the stage in survey

research when survey questions and questionnaires are tested on
members of target population/study population, to evaluate the
10 | P a g e

Downloaded by EDA MAE CONDE (edamaetabangconde@gmail.com)

lOMoARcPSD|29291824

reliability and validity of the survey instruments prior to their final

distribution. Pretesting is widely regarded as indispensable in survey
questionnaire development and is also crucial to improve data
collection for quality-of-life research. It incorporates a variety of
methods or combinations of methods.” Testing provides a means to
check whether the questionnaire is valid in itself.
A pilot survey is a strategy used to test the questionnaire using
a smaller sample compared to the planned sample size. In this phase
of conducting a survey, the questionnaire is administered to a
percentage of the total sample population, or in more informal cases
just to a convenience sample.
Setting your goals is the comprehensive view of your
questionnaire. The response from the pre-testing that shows more
answer of I do not know or poor responses to open-ended question,
multiple write-in comments on scaled questions, and incomplete
forms can suggest needed improvements.

Acknowledgment

The Schools Division of Zambales would like to express its heartfelt

gratitude to the following, who in one way or the other, have contributed to the
successful preparation, development, quality assurance, printing, and distribution
of the Quarter 2 Guided Learning Activity Kits (GLAKs) in all learning areas across
grade levels as a response to providing the learners with developmentally-
appropriate, contextualized and simplified learning resources with most essential
learning competencies (MELCs)-based activities anchored on the principles of
guided learning and explicit instruction:

First, the Learning Resources (LR) Development Team composed of the

writers and graphic artists for devoting much of their time and exhausting their
best efforts to produce these indispensable learning kits used for the
implementation of learning delivery modalities.

Second, the content editors, language reviewers, and layout evaluators

making up the Division Quality Assurance Team (DQAT) for having carefully
evaluated all GLAKs to ensure quality and compliance to DepEd standards;

Third, the Provincial Government of Zambales, for unceasingly extending its

financial assistance to augment the funds for the printing of these learning
resources for use by learners and parents at home;

Fourth, the teacher-advisers and subject teachers, in close coordination

with the school heads, for their weekly distribution and retrieval of the GLAKs and
for their frequent monitoring of the learners’ progress through various means; and

Finally, the parents and other home learning facilitators for giving the
learners the needed guidance and support for them to possibly accomplish the
tasks and for gradually helping them become independent learners.

To deliver learning continuity in this challenging circumstance would not be

possible without your collective effort and strong commitment to serving our
Zambaleño learners.

Again, our sincerest thanks!
