Motivation: SCAVENGER HUNT: Using the Assemblr Edu App the class will look around the room
using the QR codes to answer the following questions:

WHO IS THE:

2. Most admired young entrepreneurs in Philippines
3. Most difficult subject according to MCRIS students
4. Most effective way of preventing the spread of COVID -19
5. Most read book of all times

After the activity the teacher ask the following questions:


1. How do you think people come up with the answers above?
2. How will you gather information from people about any topic or issue from a big number of
people or groups?

E. Presentation of the Objective:

At the end of the 1 hour session, learners are expected to:


1. Discuss how to make a survey questionnaire.
2. Design a survey questionnaire needed for their community survey.
3. Appreciate the value of being truthful and objective.

i. ACTIVITY:

Provide hand-outs to students with sample questions.

Consider yourself as a client to answer the set of survey questionnaire. Select which question is clear
and understandable and it does not create any difficulty and confusion and which is difficult and
confusing.

ii. ANALYSIS:

What are the ways of making a well-designed and well-constructed survey questionnaire?

iii. ABSTRACTION:

A survey questionnaire should…

1. not use words that have clear meaning;

2. cover all possible options;

3. not ask two or more questions in one sentence;

4. provide and out-option;

5. consider appropriates reference;

6. have clear question structure;

7. have open specific response options;

8. not have any bias or prejudice;


9. not include too personal or too embarrassing questions;

10. avoid double negative questions;

11. outline instructions or directions clearly and understandably.

iv. APPLICATION:

Suppose you are chosen as a youth representative of your barangay and you are asked to introduce
your advocacy. Choose between the following:

1. Use of Organic Fertilizer in Farming

2. Collective Efforts against COVID-19

3. Continuing Education

Decide which advocacy you will uphold and be able to find out possible response of your town
people about it. In order to determine their attitude towards your advocacy, conduct a survey by
framing a survey questionnaire. Consider the guidelines in your five-question survey to administer in
your respective barangay.

IV. EVALUATION:

Directions: Consider the following questions, analyze the questions using the guidelines on what
should and should not be done for an effective questions. Be able to determine whether the
question is effective of not (mark check). Then give the reason
