AFFIDAVIT OF LOSS

I, PATROLMAN IBRA LARANJO MAMA, active member of the Philippine

National Police, of legal age, married and a resident of Bito, Marawi City, after
having been duly sworn to an oath in accordance with the law do hereby
depose and say:

1. That on June 17, 2020 Wednesday, I found out that my wallet and
one OPPO cellphone were lost;
a) In my wallet there are my PNP I.D, PHILHEALTH, I.D,
DRIVER’S LICENSE I.D, VOTER’S I.D, LANDBANK ATM Card
and some important receipts; and
b) A cash money worth of eight thousand (Php. 8,000.00)
pesos;
2. That despite efforts exerted in looking for, it was nowhere to find and
were totally lost;
3. That I am executing this affidavit to attest the veracity of the foregoing
facts and in support for my letter request for reissuance to PNP
headquarters, PHILHEALTH field Office, LTO Office and in the LAND
BANK of the Philippines to issue me again another identification cards
to the above mention offices;
