AFFIDAVIT OF CONFIRMATION AND RATIFICATION

We, AAA, BBB, and CCC, all of legal ages, Filipinos, and with postal address
at Kalawisan, Lapu-Lapu City, Cebu after having duly sworn to in accordance with
law hereby depose and state:

1. That we are the heirs of our late Grandfather DDD, who is the grandfather
of EEE;

2. That we are the biological children of EEE;

3. That EEE is the biological child of the late FFF, who is the daughter of the
late GGG;

4. That an extrajudicial settlement of estate was executed by the Heirs of


GGG;

5. That we were abandoned by our father, EEE, when we were still young;

6. That we have not been in any communication with our father and does
not have knowledge of his current whereabouts;

7. That we are executing this affidavit to confirm that we are the biological
children of EEE who is the grandchild of the late GGG and for all legal
intents and purposes as required by law.
