AFFIDAVIT OF RECOGNIZANCE

I, __________________, of legal age, Filipinos and resident of Brgy. Damires,

New Lucena, Iloilo, after having been duly sworn in accordance with law, hereby depose
and state, THAT:

1. I am a Barangay Kagawad of Brgy. Damires, New Lucen , Iloilo, Philippines;

2. I personally know _________________________ who is the accused in

Criminal Case No. 14-74517 for Frustrated Murder, before the Regional Trial
Court, Branch 34, Iloilo City;

3. I know that he was acquitted on December 16, 2021 by reason of Insanity and
that he was ordered to be confined at a Mental Health Facility;

4. Likewise, I know that he was recommended not to be confined anymore as he

only needs to take his medication regularly and to have a monthly follow up
consultation;

5. I am not related to him, by affinity or consanguinity, within the fourth degree;

6. Upon the grant of his motion to release, I undertake to:

a) Take custody of his person and be responsible for his appearance in the clinic
of the psychiatrist of his choice for his monthly consultation or whenever
required;
b) Present the accused before the Department of Health Mental Facility upon
the recommendation of his psychiatrist and/or if necessary;
c) Submit a monthly report to this Honorable Court of the compliance of his
monthly consultation/ check-up;

7. I execute this affidavit to attest to the truth of the foregoing.
