CONSENT AND RELEASE FORM
I, _________________________, in consideration of undergoing a tattoo and/or piercing procedure in
Siningtinta Tattoo Studio (Siningtinta Art Space) on ______________, confirm the following:
● I am of legal age and I am fully and legally capable of consenting to the tattoo and/or piercing procedure.
● My body is well-conditioned to obtain a tattoo and I am not under the influence of drugs or alcohol for
24 hours prior to session, nor am I pregnant or nursing.
● I am voluntarily submitting to be tattooed and/or pierced in Siningtinta Tattoo Studio without duress or
coercion.
● I have no communicable disease that may endanger the health of any person of Siningtinta Tattoo
Studio.
● I understand that Siningtinta Tattoo Studio cannot identify whether or not I have physical conditions that
render me unfit to undergo a tattoo and/or piercing procedure, and that I should inform my artist about
these possible conditions.
● I understand that I have to consult with my dermatologist regarding my skin conditions prior to getting a
tattoo and/or piercing, and that I do not hold my artist nor the shop liable for any medical emergencies
arising from tattooing.
● I have read the pre-session preparations and the aftercare guide. I understand that I need to follow such
instructions, and that not doing so may cause infection and medical complications among other risks.
● I acknowledge that, by obtaining a tattoo, infection is a possible risk and as such, I agree to let my artist
know if it happens and to follow instructions concerning the care of my tattoo while it is healing, or to
seek medical help.
● I acknowledge that, by obtaining a piercing, there is a possibility of feeling dizzy or lightheaded, swelling,
redness, among other risks and as such, I agree to let my artist know if it happens and to follow
instructions concerning the care of my piercing while it is healing, or to seek medical help.
● I understand that I will be tattooed and/or pierced using appropriate instruments, sterilizing techniques,
disposable and sterilized needles and original vegan US inks.
● I acknowledge that I have asked questions about obtaining a tattoo and/or a piercing and upon asking, I
have been provided answers to my full and total satisfaction.
● I understand that this procedure causes permanent change to my skin and body.
● I affirm that I am comfortable and I allow my tattoo and/or piercing to be photographed and used for
shop portfolio and be posted online. Otherwise, I understand that I should let my artist know.
● I acknowledge that Siningtinta Tattoo Studio does not allow refunds on payments made upon booking
and that reschedules are subject to separate reservation fees, unless such reschedule is caused by
events beyond the entity’s immediate control, including, but not limited to operational restrictions,
weather conditions, etc.
● I understand and affirm that I, and my household members, do not have and am not experiencing
symptoms of COVID-19 within the last 14 days.
● I have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 30 days.
● I affirm that I have not traveled outside of the country within the past 30 days.
● I understand that the Department of Health (DOH) has stated that there is evidence that persons with
COVID-19 may be asymptomatic or pre-symptomatic, and that the virus may be transmitted to the
patient or others by me if I am such a carrier.
● I understand that I am being allowed access to Siningtinta Tattoo Studio during a pandemic and that I
must follow all mentioned rules, protocols and requirements in siningtinta.com and in the confirmation
of my appointment for my safety and that of others.
● I understand that despite adhering to all precautions, there is still a possibility that I will be exposed to or
contract COVID-19 during, but not limited to, travel to the location and use of common spaces.
I am consenting to undergoing a tattoo and/or piercing procedure under the care of my artist during a
pandemic and I am WAIVING AND RELEASING to the fullest extent permitted by law any person of
Siningtinta Tattoo Studio from all liabilities, claims, and possible damages.
