“Incompetent Cervix”

Description:

 An incompetent cervix also is called cervical insufficiency, is a condition of

pregnancy whereby the cervix dilates, without pain (like not getting contractions),

classically during the middle three months of pregnancy. An incompetent cervix

happens when weak cervical tissue causes or plays a part in a premature birth or

the loss of a healthy pregnancy.

Diagnostic Test:

 The pregnancy care provider will ask questions about you health history

and pregnancy symptoms at prenatal appointments

 The provider may want to monitor your cervix more closely based on

those risk factors

 Be sure to let them know if you’ve had a miscarriage or surgery on your cervix

 Transvaginal Ultrasound

o an imaging test using a wand inserted into the vagina. This test allows

your health care provider to measure the length and opening of your

cervix.

Medical Management

 PROGESTERONE SUPPLEMENTATION

o If you have a short cervix with no history of a preterm birth, vaginal progesterone

may lower your risk of having your baby too early. This medicine comes in the

form of a gel or a suppository that gets placed in the vagina each day.

 CERVICAL CERCLAGE

o During this procedure, the cervix is stitched tightly closed. The stitches are taken

out during the last month of pregnancy or just before delivery. You may need a

cervical cerclage if you are less than 24 weeks pregnant, you have a history of

early births and an ultrasound shows that your cervix is starting to open.

 PESSARY

o A device called a pessary fits inside the vagina and holds the uterus in place. A

pessary may help lessen pressure on the cervix. But more research is needed to

see if a pessary will work well for treating an incompetent cervix

Nursing Management

 Give family and client instruction. Explain the issues that need to be reported

right away, including pink-tinged vaginal discharge, elevated pelvic pressure, and

ruptured membranes.

 Keep things in a way that protects the pregnancy's integrity.

o If necessary, be ready for cervical cerclage.

o Continue to follow the recommended activity limits.

o Talk about the necessity of vaginal rest (no sexual activity or orgasm)

 In case the membranes rupture, get ready for the birth.

 Attend to your psychological and emotional requirements
