TASK SHEET 4.1.

TITLE:
Monitoring and Recording Pulse Rate

PERFORMANCE OBJECTIVE:

Given the needed materials, monitor and record the pulse rate of the client.

MATERIALS:

● watch or clock with seconds hand


● alcohol
● ballpen
● monitoring sheet

STEPS/ PROCEDURE:

1. Perform hand hygiene.


2. Make sure that the client is at rest before starting to monitor his/her pulse rate.
3. Select the place where the pulse will be measured. It can be through the radial artery found on the
inside of the wrist closest to the thumb, on the inside of the elbow (brachial artery), behind the knee
(popliteal artery), or the neck (carotid artery).
4. Count the pulse for 60 seconds until the seconds hand returns to the 12, or count for 15 seconds and
multiply it by 4 to calculate beats per minute.
5. Record the client’s pulse rate in the monitoring sheet.
6. Perform hand hygiene.
