I would like to request from your good Office to permit us conduct limited face-to-face End of School Year
Rites on June 27, 2022 in the morning for Completion and Graduation Ceremonies and in the afternoon
Recognition Program at Baracbac National High School Social Hall.

The said program is a compliant of DepEd Memorandum no 043, series 2022 which warrants us to conduct a
limited face-to-face EOSY Rites with observance of DOH and IATF protocols after consultation with our
Municipal Mayor as our Local Chief of Executive.

Attached herewith ore letter to the Mayor, Parents’ Consent and List of Graduate and Completers.
