RESOLUTION REQUESTING HONORABLE PUNONG BARANGAY OSMUNDO S. ABUNGAN, SR. OF

BARANGAY NANCAPIAN, MALASIQUI, PANGASINAN TO EXTEND ONE CIVIC VOLUNTEER OFFICER
EVERYDAY STARTING MARCH 28, 2022 FOR THE EXPANDED LIMITED FACE TO FACE LEARNING
MODALITY OF NANCAPIAN NATIONAL HIGH SCHOOL, NANCAPIAN, MALASIQUI, PANGASINAN

WHEREAS, Nancapian National High School will start its Expanded Limited Face to Face Learning
Modality on March 28, 2022;

WHEREAS, the school needs to implement the guidelines stipulated on the Face to Face protocols
set by DepEd and IATF for the said Limited Face to Face Learning Modality;

WHEREAS, the said Learning Modality needs to have a personnel in-charge for traffic in front of
the school gate;

WHEREAS, in motion of Faculty Club President, Mr. Renato G. De Luna and duly seconded by
Mrs. Jona T. Cayabyab; it was,

RESOLVED AS IT IS HEREBY RESOLVED TO EARNESTLY REQUEST Punong Barangay Osmundo

S. Abungan, Sr. of Brgy. Nancapian, Malasiqui, Pangasinan to approve this resolution to send one CVO
from Monday to Friday -7:00 a.m. to 8:30 a.m. only and 11:45 a.m. to 1:00 p.m. only from Mondays to
Fridays to Nancapian National High School;

RESOLVED FURTHER that copy of this resolution be furnished to the Office of the REQUEST
Punong Barangay Osmundo S. Abungan, Sr. for his favorable action and kind consideration.

RESOLUTION REQUESTING HONORABLE PUNONG BARANGAY OSMUNDO S. ABUNGAN, SR. OF

BARANGAY NANCAPIAN,MALASIQUI, PANGASINAN TO ALERT HIS BARANGAY BHW PERSONNELS
OF NANCAPIAN, MALASIQUI FOR THE FACE TO FACE OPERATION OF NANCAPIAN NATIONAL HIGH
SCHOOL STARTING MARCH 28, 2022;

WHEREAS, Nancapian National High School will start its Expanded Limited Face to Face Learning
Modality on March 28, 2022 as approved by SDO1 Pangasinan and LGU of Malasiqui;

WHEREAS, the school needs to implement the guidelines stipulated on the Face to Face protocols
set by DepEd and IATF for the said Limited Face to Face Learning Modality;

WHEREAS, the said Learning Modality requires the partnership of the Local Barangay Nancapian
and the school- NNHS in case there exist a student with above normal temperature;

WHEREAS, if in case the alleged student got high temperature at the entrance of school, the officer
in charge will inform the adviser; then the adviser informs the school Nurse; then the nurse informs the
DRRM Coordinator and finally the DRRM Coordinator will connect to the BHW of the barangay for proper
treatment;

WHEREAS, in motion of Faculty Club President, Mr. Renato G. De Luna and duly seconded by
Mrs. Cecile V. Launico; it was,

RESOLVED AS IT IS HEREBY RESOLVED TO EARNESTLY REQUEST Punong Barangay Osmundo

S. Abungan, Sr. of Nancapian, Malasiqui, Pangasinan to approve this resolution to send one BHW per day -
7:00 a.m. to 8:30 a.m. only and 11:45 a.m. to 1:00 p.m. only from Mondays to Fridays to Nancapian
National High School;

RESOLVED FURTHER that copy of this resolution be furnished to the Office of the Punong
Barangay Osmundo S. Abungan, Sr. for his favorable action and kind consideration.
