To Whom It May Concern:

I am writing today to complain about a car that I saw having the same plate number with our
TOYOTA FORTUNER 2016. I was able to visit one of your regional offices in NCR,
specifically the one near Quezon Avenue and they were able to check that our plate number is
legitimate and has no issues with it.

The person I was talking to gave us an advice to visit the Main Office and accomplish this letter
along with photos to prove our complaint.

Here are the details of our vehicle:

Plate Number: NDB 5600

Vehicle type: SUV

Make: Toyota

Model: Fortuner

Year: 2016

We saw the car with the same plate near our place and we were also able to get the contact
number of the person riding the car. We are very concerned with the current situation as there
might be incidents with the car with the same plates and it gets charged to us.

I will attach photos of the car and our documents.

I would welcome the opportunity to discuss matters further and to learn how you propose to
prevent a similar situation from recurring. I am looking forward to get these matters resolved as
soon as possible at this post’s real danger to me and my family. I look forward to hearing from
you.
