Gerona, Tarlac, after having been duly sworn in accordance with law, hereby, depose and say:

1. That I am the Chairman of the Board of PGT Transeco, duly organized and existing
under the Cooperative Development Authority and laws of the Philippines with office
address at # 298 Brgy. Samput, Paniqui, Tarlac.

2. The type of cooperative is Small and Midsize Enterprise (SME) and the principal
business is credit for financial intermediation to members for productive and providential
purpose.

3. The cooperative is only transacting in members and employee only.

4. That the amount of the accumulated reserves as of the year end is Seven Thousand Two
Hundred Three and Ninety Three Cents (Php 7,203.93.00)

5. That the amount of net surplus for the year end is Fourteen Thousand Four Hundred
Seven and Eighty Six Cents (Php 14,407.86.00)

6. That the 25% of net surplus is Three thousand six hundred One and ninety six cents (Php
3,601.96) divided into forty eight members.
