Accident Report

What is an Accident?

 An Accident is an unplanned event that results in personal injury or property damage. This

could include:

1. Sprains: ( ‫ )إلتواء‬Overstretching or tearing the ligaments results in a sprain. Ligaments are

pieces of tissue that connect two bones to one another in a joint.

2. Strains:) ‫)عضل شد‬Overstretching or tearing muscles or tendons results in a sprain. Tendons

are thick, fibrous cords of tissue that connect bone to muscle.

3. Knee injuries: ( ‫الركبة‬ ‫ )إصابة‬Any injury that interferes with how the knee joint moves.

4. Swollen muscles: (‫ ) العضالت تورم‬Swelling is a natural reaction to an injury.

5. Fractures: (‫ ( كسور‬Are also known as broken bones.

6. Dislocation: (‫( انخالع‬When that happens, a bone is forced out of its socket. This can be painful

and lead to swelling and weakness.

7. Concussion: )‫(صدمه‬ a violent shock as from a heavy blow to the head.


What is an Accident?

8. Unconsciousness: )‫أال‬ ‫ ) وعي‬not awake and aware of and responding to one's environment.

9. Lacerations: ( ‫ (تمزق‬a deep cut or tear in skin or flesh.

10. Ill-health or Sickness: )‫( الصحة اعتالل‬due to exposure to a dangerous substance, fumes or gases, fire

or explosion.

11. Sickness: (‫( مرض‬due to a chemical spill or environmental pollution.


What is the difference between an accident
report and an incident report?

 An accident report focuses on the injury.

 An incident report is intended to focus on the cause and prevention of


further occurrences.
What is an accident report form?
 The Accident Report Form is in 2 parts. The first part is completed by the employee who suffered the
accident, and the second part is completed by the supervisor.

 An accident report is an important tool used to document the accident and assist in investigating the
cause. It also assists to develop procedures that may be put in place to prevent it from happening again.

 The supplied template calls on the employee(s), with the assistance of a manager, to fill in the first page
details including:
✓ employee details
✓ injury details- including date, time and expected time off work
✓ medical treatment required
✓ events leading up to the injury- this is important to gain the employees perspective of what actually
happened.
What is an accident report form?
 The second section of the form is to be completed by the manager and requires the manager to
identify the following:
✓ witness details
✓ how the accident happened
✓ how a recurrence can be prevented.

It is important to show outcomes of the investigation and to document what actions can or will be
taken to prevent another injury occurring in a similar scenario

The Manager should decide whether or not the accident should also be documented as an incident
needing further investigation.

Should your company decide to merge both the accident and incident form together into one
document then the suitability of this should be assessed at senior management level to ensure that a
detailed and documented approach to both matters is still achieved.
