I. ELIGIBILITY DOCUMENTS

One set original / certified true copy and four sets machine copies

a. Certified true copy of Registration certificate from securities and exchange commission (SEC),
Department of trade and industry (DTI) for sole proprietorship

b. Certified true copy of mayor's permit or business permit issued by the city or municipality where the
principal place of business of the proponent

C. Certified true copy latest income tax return

d. Certified true copy latest audited financial statement showing among others, the suppliers total and
current assets and liabilities, stamped "received" by the BIR or it's duly accredited and authorized
institutions; or certified true copy of email showing the system generated eAFS transactions reference
number, for the preceding calendar year which should not be earlier than two years from the date of bid
submission

e. Bid Bond

f. Original Notarized secretary's certificate or special power of Attorney for authorized representatives

g. Original of duly signed instructions to proponents [Format here in attached and referred to as "FORM
A"]

h. Original Certificate of inspections signed by GA SAO [format here in attached and referred to as
"FORM B"]

i. Certified true copy of the official receipt for the payment of bidding documents issues by GA, DND

ADDITIONAL REQUIREMENTS

1. Each sealed envelope must be properly marked / labeled

2. All documents to be submitted must be properly tabbed.

3. Improper /wrong placement of documents in the specified envelope shall result to proponents
disqualification.
